How Long Does Cocaine Stay in Your Hair? The Borderline Cases

A standard US hair drug test analyzes the 3.9 centimeters (1.5 inches) of scalp hair nearest the root, which at Quest Diagnostics' published growth rate of about 1.3 centimeters per month covers roughly the previous 90 days. Cocaine does not clear from hair the way it clears from blood; once incorporated into the growing shaft it stays until that hair is cut or shed. The reported window is the length of hair a laboratory agreed to cut, divided by an assumed growth rate. Two edges bound it: use in the past 5 to 10 days has not yet grown above the scalp, and the proposed federal guidelines would test only 0.5 to 1.0 inch, roughly 30 to 60 days.

Where the ninety days comes from

That number is arithmetic on two published figures.

Quest Diagnostics asks collectors for about 100 mg from the crown, the first inch and a half from the root end, roughly 120 strands. The lab tests that 3.9 cm and discards the rest. Quest puts crown growth at approximately 1.3 cm per month. Divide 3.9 by 1.3 and you get three months.

Use the other published rate and the same segment covers a different span. The proposed federal Hair Mandatory Guidelines, at 85 FR 56108 (September 10, 2020), put average vertex growth at about 0.4 millimeters per day, approximately 1 centimeter per month. At that rate 3.9 cm is 117 days. Neither figure is wrong; both are averages over hair that grows at different speeds in different people and across one scalp. That 30 percent spread rides inside every "90 day" claim.

The federal document proposes a shorter piece: 0.5 to 1.0 inches, approximately 2.5 cm, "representing a detection time period of approximately 30-60 days," for pre-employment and random testing only. It adds that drugs are not detected in hair for 5 to 7 days after ingestion, ruling it out for post-accident and reasonable-suspicion testing. Quest puts that blind period at 5 to 10 days.

Those guidelines were never finalized. HHS lists its certified laboratories monthly, and the September 1, 2026 notice at 91 FR 56152 covers urine and oral fluid only. No US laboratory is HHS-certified for hair, and the Department of Transportation has not approved it. Your test ran on the laboratory's own accreditations and your employer's policy.

What the laboratory is actually measuring

Cocaine is the target analyte. Its metabolites decide what the number means.

Quest screens the cocaine group at 500 pg/mg and confirms cocaine, benzoylecgonine, cocaethylene and norcocaine at 500 pg/mg each, by GC-MS or LC-MS/MS. The federal proposal keeps the 500 pg/mg initial cutoff and confirms cocaine at 500, but sets benzoylecgonine at 50 pg/mg, one tenth of the commercial threshold. The 2021 Society of Hair Testing consensus also puts cocaine at 500 pg/mg and requires benzoylecgonine, norcocaine, cocaethylene or the hydroxy-metabolites to confirm use.

Metabolites carry that weight because the parent drug proves contact, not consumption. The federal preamble states that benzoylecgonine in a user's hair "can be from drug ingestion and/or external contamination." Cocaine powder contains benzoylecgonine as a degradation product, so handling it deposits the metabolite directly.

One borderline goes unflagged almost everywhere. The Society of Hair Testing applies its cut-off to segments no longer than 3 cm and warns that dilution must be considered beyond that. The commercial standard tests 3.9 cm as one piece, 30 percent past what the cut-off was written for, averaging a light pattern down across it.

I have never worked a bench in a toxicology laboratory. I cannot tell you how a wash-solution chromatogram is read, or how a lab decides a strand is porous enough to flag. What I can vouch for is what reaches a claim file: analyte list, cutoff column, specimen-source line, and whether those agree with the window somebody typed into a narrative.

Scalp hair and body hair are two different records

The federal proposal limits collection to head hair, citing "different growth rates and drug detection windows based on the location of hair on the body."

Commercial laboratories accept body hair, and this is where files go wrong. Quest's order of preference for a donor with no head hair, or hair under half an inch, is chest, underarm, leg, then facial, with the source noted on the envelope and never mixed with head hair. Because body hair grows to a set length and then stops, Quest states that "one cannot reliably determine the window of detection of drugs using hair from alternative body sites."

The first hair-test file I reconciled, I logged the specimen as a standard 3.9 cm scalp collection because the requisition said "standard." The report said underarm. I had built a 90-day chronology on a specimen that supports no defensible window, and it came back marked in red with the hearing already scheduled. We withdrew the chronology and refiled.

Hair against urine and oral fluid

These three answer different questions, though search summaries line them up as three lengths of one ruler.

Those oral fluid guidelines took effect on October 10, 2023, yet the September 2026 HHS notice still reads: "there are no laboratories certified to conduct drug and specimen validity tests on oral fluid specimens."

A negative hair test alongside a positive urine test is not a contradiction. It is what you expect when use falls inside the blind period.

Whether hair color changes the result

Cocaine binds to melanin, and the federal preamble gathers the evidence. In vitro work found up to 34-fold differences in cocaine binding between dark and blonde hair; a human study dosing isotopically labeled cocaine found roughly 2.7 times more incorporated into non-Caucasian than Caucasian hair. In a codeine study at a 200 pg/mg cutoff, every black-haired subject and half the brown-haired subjects would report positive, while blond and red-haired subjects on the same dose tested negative. In Jones v. City of Boston, the First Circuit found sufficient evidence of disparate impact in Boston police hair-test data from 1999 to 2006.

Bleach, dye, relaxer, and the claims built on them

Chemical treatment does affect results, and the direction of the error is the opposite of what people assume.

The Society of Hair Testing lists bleaching, dyeing, perming, straightening and UV exposure as influences on hair drug concentrations, and says treatment type should be documented at collection. The federal preamble adds that such treatment may increase susceptibility to environmental contamination. Cuypers and colleagues, in work cited in the federal proposal's own reference list, bleached hair from cocaine users with a commercial 9 percent hydrogen peroxide product and found all evidence of cocaine use removed after 15 minutes.

Here is the strongest case against me. Quest reports that bleaching and dyeing applied to cut samples under worst-case conditions "typically did not interfere with the test results or change the reporting category." That is an accredited laboratory running these assays at volume, heavier evidence than imaging a few strands. I grant it. At a 500 pg/mg confirmatory threshold, a heavy-use specimen has room to lose signal and still report positive. The disagreement bites only near a cutoff, hardest at the federal proposed 50 pg/mg, and HHS did not treat it as settled either, proposing that every laboratory hold a validated method for identifying damaged hair.

So a negative from bleached hair is a weaker negative. A positive from bleached hair was not created by the bleach.

The rest of the advice fails on simpler grounds. Quest reports that shampoos, sprays and gels had little or no effect, and that no adulterant it has tested beats the assay. Every specimen is washed before analysis anyway. Shaving produces a collection problem: specimens under 1 cm are rejected outright, and a donor with no usable head hair is routed to body hair, which carries no reliable window.

How to read a result without inventing a date

A hair result supports a narrower claim than people read into it.

The Society of Hair Testing states that concentration cannot determine the quantity consumed. Quest answers "No" to whether segments should be analyzed for specific periods of use, because growth rate is not constant between or within individuals, and holds that segmental analysis should not identify intervals "in a forensically defensible manner." Nothing in a result speaks to impairment; the shaft tested grew days after any blood concentration was gone.

Through about 2022 I told claimants to write out a dated timeline of the 90 days before collection. I stopped. It produced a document that invited everyone to argue over dates the specimen could never establish. Now I advise recording what the record carries: collection date, specimen source, segment length, assumed growth rate. The consensus does allow segmental analysis to describe a pattern. Pattern and date are different claims.

The documents worth keeping

This is what arrives in files.

  1. Confirm the specimen type and form in writing. Hair uses an Alternative Specimen Custody and Control Form, not the urine CCF, whose seal is too long for the hair envelope.
  2. Have the collector record the source site on the envelope, and state any coloring, bleaching, relaxing or perming in writing.
  3. Ask, in writing, what segment length the laboratory will analyze and what growth rate it assumes.
  4. On a reported positive, start a 72-hour clock. The federal proposal gives a donor 72 hours from Medical Review Officer notification to request testing of the split B specimen. Non-federal programs are not bound by it, so request the employer's policy that day.
  5. Keep prescription and treatment records with dates attached.
  6. Watch retention. Quest holds non-negative hair specimens a minimum of 12 months and testing records two years. A dispute outliving those has no specimen left to retest.
